Please be advised Wellesley Management has updated its
Work Order Form procedure for all tenants. Starting Wednesday, January 16 please utilize Wellesley’s
Work Order Form for all suite and common area maintenance requests. Enclosed in the document is a sample Work Order Form for your reference. Below are easy-to-follow instructions on how to complete and submit the Work Order Form to Wellesley Management. Should you have any questions, please contact Marc Fantasia at 978-823-8226.
- Download and print the Work Order Form by clicking here.
- In the top-right boxes, please write the date, mark whether the request is an emergency, and mark the area where the maintenance request is located.
- Please complete all of the top areas, being sure to write legibly. Please note: The contact person must be available to meet with a maintenance supervisor between the hours of 9am and 5pm for 5 business days following the submission of the Work Order Request. Failure to provide access or explanation will result in delay of service and/or additional charge for tenant “no-show” visits.
- Please mark the type of maintenance request(s) in the corresponding box(s). For example, if you are experiencing a lack of heat in your space, please check “No Heat” under the “HEAT” section in the “HVAC” box. If your request does not correspond with one of the line items, check “Other” and provide a description of the maintenance issue in the “DESCRIPTION” box.
- If necessary, provide further details about your maintenance request in the box marked “DESCRIPTION”.
- Once complete, FAX the Work Order Form to Mary Ellen Lorion at 978-823-8202 (phone: 978-823-8200). The next Wellesley Management service person will respond to the contact person the next business day or as soon as possible. Please note: Please be sure to completely fill out the request. Any incomplete areas will result in a delay in processing and service and/or additional charges.
